| Applications accepted | Availability of space. before children are admitted to the cdc, parent/guardian(s) pay a nonrefundable enrollment fee and complete admission materials including: enrollment form parent agreement appointment of agent form certificate of health assessment child care food program (cacfp) income data form notarized custody declaration (when applicable) a certificate of physical examination (performed within the last six months), a written physician's release for children under the age of 6 weeks, and a complete record of the child's immunizations are required by the cdc prior to the child's first day of attendance. |
