| Applications accepted | Each year applications for the lottery will be open from february 1 to march 30. the following information and documentation for the applicant is required by the state of texas and must be included in this application: social security card - you can contact or go to the social security administration to apply for new or recover lost cards. birth certificate - obtain from the county/state/country where your child was born. they will have their certificate on file which cane be ordered by phone or on-line. immunization record - up to date immunization records are required for all new students. each new entering student must also have a health form and immunization record completed by a physician. returning students must have their immunization boosters and submit new updates to the school. you can visit the tarrant county immunization site here. |
