| Applications accepted | Immunization and health certificate, ? emergency card, ? copies of custody/guardian decree, progress and attendance reports, testing scores, evaluations, medication and accident reports. children may begin school no sooner than their third birthday. should an applicant receive a notification of non-acceptance, the deposit will be refunded within three weeks of notification. admission to cma shall conform to the constitution and applicable laws of the state of ohio. directresses shall interview each prospective parent and child. |
